Send Portal Link / Review to see if Portal History was completed

Set up of the Electronic Forms

Under Utilities - System Set up - Patient - Forms - you can mark the box Active for the forms you wish to send via the portal or to the Intake. (Kiosk)  You have the ability to customize each forms verbiage.

Click on New Patient - fill in info - Click Send New Patient Link and Create Patient

Patient must have a Email to Send New Patient Link

Another way to send Link - Search and pull up the patient - click on Wand and Edit Information - click Utilities - then Send Patient Portal Link

Click Send Patient Portal Link - Confirm email address and OK

Check mark all information you would like collected.  Uncheck all options if you are just granting access to patient portal.

If you wish to wait for the patient to complete the forms in the office, once they arrive you can send them to the patient Kiosk for the patient to complete.

Email Sent to Patient

Patient will click "here" to create a portal log in and fill in the health history

Patient will enter in Email address and create a Password

Log in and under My Task List click Go To Health History

Patient will fill out Health History

When Health History is Completed 100% Patient will see "Thank you! We look forward to seeing you."

Each form you will click to access where the patient can enter their signature.  Once the document is signed, it will appear in the patients file vault.

How to remove form from Kiosk/Portal that was assigned

Go to Edit info, Utilities, Patient Portal

Scheduler

Green = Patient has Opened/Started and completed the New Health History via the patient link sent to them.  

Light Yellow = Patient has been sent information via the patient link but has yet to complete the New Health History.

How to see if Health History was Completed

Click on Patient Wand and Doctor Icon

Audit Log - show if Email was sent for Welcome to Our Clinic, or Patient Portal Access Link

Patient Log - Shows if New Case History is Completed.  If Date is Blank, you want to add the Date of NP Appointment

Click on Date Box - Choose the Date of NP Appointment and hit OK

When a patient is Green you will see that it has a date and a check mark under Complete.

When patient is light yellow on the schedule you will see in the Patient Log that it shows "incomplete" under the description.

If there is no date and shows Incomplete for a New Case History and it needs to be deleted, you can right click and Delete Case History.
