Documentation that either has been shared outside the office OR is more than 72 hours old should only be changed through an Amendment. A patient may also request their documentation to be amended which would be done in the same way.
Adding information to a Document
The other option would be if you need to update a document and want to leave the original verbiage, you can use the Strike Thru to show the changes.
If the patient requested the amendment but the Doctor does not agreed with their request, type their request in the text box, select Denied and then Save without modifying the documentation.
