If you have a carrier where they require different billing information on a claim you may need to add a second Entity, Facility and Provider. This is because it will change the information for ALL insurances which is why a second Entity, Facility and Provider is added.
Adding Entity



1. Code: You can use any Code you wish to decipher which Entity you are using if you have to choose.
Company: Put your Company name as appears for your Tax ID / 1099
First, MI, Last, Suffix: Fill out the name that is registered for your Tax ID / 1099
2. Physical Address: Put your location of your office including the 4 extra digits of your zip code
You can look at mail that has been sent or your can google your office address with the post office to get the last 4 digits of your Zip Code.

1. You can click the box for the Pay to Address to be the Same as Physical Address or you can type in a different address.
2. You can use any email address and contract in this space. Typically the Doctor / Owner or Office Contact.
3. Entity Type: From the drop down select Non- Person Entity
Tax ID Type: Use EIN if you are using a Tax ID or select Person if you are using a Social Security.
Tax ID: You will fill out your group Tax ID (no space no dash) if you are using an EIN. Or you will type in the Social Security number for Person
4. Taxonomy Code: You will use the Code that is associated with your Tax ID.
5. EDI Type: If you are billing to your clearing house you will select the Clearing house name that is listed on the drop down.
You might have an EDI Type of CNC if you are using any carriers that require you to download a file to their website



Facility



1. Select the Entity you want the facility to be attached to.
2. Code: This can show as whatever code you would like to see from a drop down of the facility.
Name: Enter the Facility name as appears on your Tax ID/ 1099
Address: Enter the Facility address including the last 4 digits of your Zip Code.
Phone / Fax: Enter office phone and fax numbers if available
3. NPI: You will put the group NPI unless billing for an individual and you would then put the individual NPI.
Tax ID: You will put the group Tax ID (no space no dash)
Qualifier: Typically this is 77
POS: Typically this is 11
Provider


1. Entity: Choose the Entity from the drop down you want this Provider to be attached to.
Code: This is how the provider name will appear on your scheduler
Entity Type: Select Person from the drop down menu
2. Name: Add the Dr. First name, Middle Initial and Last name.
Suffix: This would show as DC for example.
Nickname: What the Dr. would like to be referred to.
Street address: Fill out the address of the clinic.
Phone numbers: Fill out your office phone and fax. You do not have to include Mobile Phone.
Email: This would be the Dr's email address
3. NPI: Enter the Dr's Individual NPI. (no space no dash)
Taxonomy code: Enter according to how the Dr. has registered their NPI.
Tax ID: This would be the clinic Tax ID.
4. Put a check in the boxes for all the items you want Dr. to be available for.
Signature: This will allow the EHR to link to the provider how they wish to sign their note in the EHR.
Initials: This would be the Dr's Initials
You typically don't mark signature, can schedule and send to EHR as you don't want the Dr. having to switch providers for their notes or schedule separately. Just needs to be marked for rendering/treating/referring.
