New Patient History Kiosk - HS 11/19/24

The Front Desk Kiosk will be used by your New Patients for their history. Your existing patients will use the Front Desk Kiosk for Check-In daily use.

Google Chrome is the recommended browser for the kiosk.  If your device does not have chrome, we do recommend that you download it for your kiosk.

Log in to the kiosk

Your User Name and Password is the same User Name and Password you use to log into the Software

Enter User Name and Password and click Log In

New Patient History or Auto Accident

Click New Patient History or Auto Accident to start the correct History

  1. Find Patient - Will Search for Patient already in the database
  2. Create New Patient (Walk In) - To add a patient not in the software
  3. Back - Go back to the last page

Find Patient

Type Last Name in Find Patient and Search

Find patient and click on Patient Name to Select

Click Next to start history

Patient will fill out Patient Information and click Next to continue

Patient will then choose Yes or No if this is a result of Auto Accident or Work Injury then click Next

Patient will choose up to 4 regions that pertain to the visit and then click Next

Patient will fill in the Complaint Information depending on the regions they picked

Any questions with a RED Asterisk must be filled in or patient can not continue

Once complaint questions are filled in patient will click next then answer questions about themselves

Once questions about the patient are completed, they will click next at the bottom then you will answer Immediate Family Health History

Click Next and you will see "Thank You! We look forward to seeing you."

If Patient does the history by the Portal Link 1st this process does not need to be done! This is only needed if History was not done before patient comes in!
