Therapy Log Basics - HSWorx

The Therapy Log details modalities and exercises, per patient, giving the therapist a checklist to track what needs to be performed. Additionally, the built-in timer keeps track of how long the patient is in therapy and the notes function provides a place to document issues that may occur during treatment. Notes are a great way to communicate with the doctor and as a tool to refer back to when evaluating a patient's progress throughout their care plan.

The Patient List

Patients appear in the Therapy Log when they are checked in. They will appear in the list in visit time order.

  1. Date - Click the date to choose a Therapy Log from a different day.
  2. Search - Enter a patient's name to select them without having to scroll through today's list
  3. Checked In Only - uncheck to see all patients, check to see only Checked In patients
  4. Treatment Plan - Click to see the selected patient's treatment plan
  5. Log Vitals - Click to Log Vitals
  6. HSMS - Click to log HSMS scores
  7. Add Patient - Click to add a patient to therapy log that is not on the schedule
  8. Play/Start - When a patient starts therapy, click the Start arrow to begin tracking time. When a patient's clock starts, their name is highlighted in green and a Pause icon appears where the Start arrow was. Click the Stop icon when the patient is finished with therapy.
  9. Hide - If a patient does not go for therapy on a particular day, click this icon to hide them from the list
  10. Length of time - Accumulated time is shown here
  11. Pencil - If you need to edit the accumulated time, click the Pencil icon

Modalities and Exercises

The panel on the right is broken down into four sections:

  1. Patient information
  2. Overview of therapy session
  3. Modalities
  4. Exercises
  1. Patient information displays the patient's account number, first name and last name initial.  The Wand makes the patient's menu available and the case displayed is the one associated with today's visit.
  2. Goals - Goals assigned the the Treatment Plan for this case
  3. Room locator - for tracking where the patient is in the clinic
  4. Active Time In and Out - These values reflect the time in and out from the timer shown in the Patient List
  5. Reset icon - Clicking reset updates the entire session as if the patient had not begun therapy. A warning box verifies the reset
  6. Initials - The therapist must initial the patient's log for the information to proceed to the Super Bill and the visit note
  7. Notes - Enter notes that describe an overarching observation about the therapy. Notes per exercise can be entered below
  8. The Therapeutic Modalities section includes details about prescribed modalities.
  9. The Therapeutic Exercises section has details about today's plan, past visits, and ways to log what happened during the patient's therapy.

Therapeutic Modalities

Normally checking the Performed box is all that is needed for logging modalities. However, there are options for adding modalities on the fly and for making notes that will explain any variation that may have occurred.

Performed box in Green means it was Performed last visit.  If Performed box is in black that means it was not performed last visit or just added.

Click Return to Therapy Log when finished adding.

Therapeutic Exercises

The Therapeutic Exercises section works the same way as the Therapeutic Modalities section, with a few additional functions.

  1. One-on-one - Click the link to change this to Group if the patient is working in a group setting
  2. Total Minutes - This value defaults to the total minutes of the prescribed exercises. Default times are established in the set-up section of the software
  3. Note - Click to add a note
  4. Up Arrow - Click this arrow to add an exercise protocol on the fly
  5. House - Click to add home exercises
  6. Printer - Click to print Exercises
  7. Clear All - Click to Clear All Exercises
  8. Photo Icon - Click the Photo Icon to see photos of the exercise
  9. Click the + sign to show prior visits

Total Minutes are estimated using default values. Enter the actual time after the patient has completed therapy.

Make notes about the patient's performance to communicate with the doctor, tell the story of improvement and support the reason for billing in-clinic exercises.

Adding Exercises from Inside the Therapy Log

Doctors may add an exercise based on the patient's current visit. Add exercises directly in the Therapeutic Exercises section of the Therapy Log.

Documenting the Visit - Overview

The value of the Therapy Log is multi-faceted, from helping the doctor plan the patient's care to providing a list for the therapist to work from to documenting the visit. Using the Therapy Log to track and document the patient's therapy is where all of those facets come together to tell the story of the visit and that patient's progress.

  1. When the patient comes to therapy, click the Play/Start on the Patient List
  2. Check the Performed box for each modality and exercise performed
  3. Make notes to describe the patient's progress or problems
  4. Click the Stop at the end of the visit
  5. Initial the visit
