WEB Front Desk Kiosk and New Patient History- HSWorx

The Front Desk Kiosk will be used by your New Patients for their history. Your existing patients will use the kiosk for daily check-in.

Google Chrome is the recommended browser for the check in.  If your device does not have chrome, we do recommend that you download it for your kiosk.

1.  Check- in w/Progress Questions: This is for all your existing patients.

2. Check- in Only: This is just to confirm the last name, DOB and demographics and check your patient in.

3. New Patient History: You will use this for your New Patients.

4. Auto Accident: You will use this for your established patient that was in an accident.

Once your office is live with the software, you will use your HSWorx user name and password to login to the kiosk for your clinic database, but you will use the following credentials for the training kiosk:

User name: train3  (not case sensitive)       Password: Training321  (make sure you use capital T as it is case sensitive)

Set Up

Make sure any changes within this screen you click the red Save at the top of the screen

In the Settings you can set up the following:

  • How many days since last visit for your Reactivation (RA)
  • If you wish to have the kiosk ask about any new injuries, accidents or fall since last visit
  • Deciding your Daily Visit Questions for Active and/or Wellness Patients, how often or for what visit types for the Subjective Progress Report Frequency for Active and Wellness Patients
  • ? Mark box allows you to determine what additional questions you want your Active and Wellness Patients to receive on the kiosk

Only the boxes that are checked will ask those questions for Active Patients on Subjective Progress Report Day

Daily Visit Questions

If Daily Visit Questions is checked that will ask the patient at Check-In the Pain Level, Pain Frequency, and Primary Pain Quality.

What patient will see in the Kiosk for Daily Visit Questions

Subjective Progress Report Frequency

What Patients will see on Subjective Progress Report days depending if you have the settings for RE Only, or Every so many Days or Visits

Check-In w/Progress Questions

Patient will click Sign In to check in for their appointment

Patient will type in Last Name and click Next

Patient will then Select the Month of Birthday and Select the Day and hit Next

Patient will be asked "Since your last visit on X/XX/20XX, have you had any new injuries, accidents or falls?" click Yes or No

If the patient selects Yes, they will receive an abbreviated New Case History which consists of complaint information only, not family or social history.

Patients enter key information for the visit during check in at the kiosk:

Daily Questions

  • VAS - Pain Level
  • Pain Frequency
  • Primary Pain Quality

Weekly

Daily questions plus:

  • VAS - Pain Level
  • Pain Frequency
  • Primary Pain Quality
  • 3 outcome assessment questions

This information lays the foundation for the visit. Make changes or notes as necessary.

Answer all the questions with red indicator and that section will collapse to answer other complaints questions

Accessing Forms to Send to Portal and Kiosk

Once Patient clicks Complete they will go to the Sign in page and either click Continue, or click on the Update Menu boxes to Edit

Once Continue is clicked they will get the "Since your last visit" which they will answer Yes (to add in a new complaint), or No (to update VAS)

Click No and patient will see the screen below and Update the VAS, Pain Frequency and Quality and then click Next

Click Next after updating patient will see "Thank you for answering these questions! Please let the front desk know you are finished.  The doctor will be with you shortly!"

Check-in Only

The Check-in Only asks the Patient Verification questions. They can Update Photo and the patients information if needed. Once the patient hits continue it will complete the check-in process.

If there are any documents that were sent to the kiosk the Check-in Only would bring them up for the patient.

New Patient History

1.  Find Patient: Allows you to search for the New Patient who has an appointment on the scheduler by their Last name and Date of Birth.

Once you click Next it will begin the New Patient paperwork. Click the link below for details.

2. Create New Patient: This button is selected if this is a Walk-In New Patient to your office.

Once all fields are filled - clicking next will bring you to the complaint part of the New Patient paperwork.  This will create the patient in HSWworx for you to search and put on your scheduler.

Auto Accident

The Patient information screen will appear if there is information missing that is required. In this example there was no Email Address.
